Kindergarten Registration Information

Welcome to our registration information page.  We are starting to pre-register our kindergarten students who will turn 5 by August 1, 2022 for the 2022-2023 school year.  To the left are forms that you can print off, complete, and mail in to us.  We are going to send these forms and a self-addressed stamped envelope to return to us if your child went to Head Start or one of our local day cares.  If you have a child who will turn 5 by August 1, 2022 and he or she hasn't been in a local day care or attended our Head Start, please fill out the forms and mail them to us at:

We are also providing you with links to additional forms that may help you as your prepare the documents you will need for the start of school.  All students will need an official birth certificate, immunization records, and preventative health care exam to start school.  They are also required to have a dental exam and an eye exam by January of their first year in school.  These forms are not required to be completed for pre-registration.
